During your first dentist appointment, you’ll need to complete insurance and health information forms to help us get started with your treatment. These forms will also help introduce you to our office.
Download and complete our patient forms before your first visit for added convenience. You can easily print and fill out the forms at home, then bring them along to your initial appointment.
During your new patient visit, we typically provide a thorough dental cleaning, take any necessary x-rays, and perform a comprehensive oral examination. This first visit allows us to understand your dental needs and goals for personalized treatment.
If you’re coming in for a dental emergency, we will focus on the tooth causing the issue. Depending on the diagnosis, you may need to schedule a follow-up appointment for more in-depth treatment.
